Standard Hotel Uniform Issue Form & Acknowledgement Template
Driving Accountability from Day One of Employment
When a new team member joins your hotel or resort, issuing their uniform is a critical step in the onboarding process. A premium hospitality uniform is a valuable company asset—blazers, custom trousers, tailored skirts, and branded chef whites represent a significant financial investment. Without a formal, signed acknowledgement at the moment of issuance, you have no legal or operational recourse if that uniform walks out the door during offboarding.
A standardized Uniform Issue Form establishes immediate personal accountability. It ensures the employee understands their responsibility to care for, clean, and ultimately return their issued garments, while protecting the hotel from procurement leakage. Below, we provide a standard, copy-pasteable checkout template recommended for hotel HR and wardrobe HODs.

The Transition to Digital Onboarding Records
While paper sign-out slips are better than nothing, they are notoriously difficult to track. Sheets get lost in HR filing cabinets, names are misspelled, and retrieving details during exit interviews is slow. Transitioning your issue checklist into a cloud-based platform like Uniformly eliminates this friction. When you scan a garment barcode to assign it, the system instantly logs a digital timestamped signature. If a staff member departs, their outstanding inventory is visible in one click, enabling water-tight offboarding exit gates across your hotel properties.
